The following application must be filled out completely before we can review your financial request. Incomplete applications cannot be processed. Every request is prayed over and reviewed by our benevolence team. You will be contacted by phone with a decision. Please note that the full benevolence process—including application review, verifications, receipt of required supporting documents, decision, and any approved assistance—can take up to 10 business days.

Please note:

1. If you do not attend Church of the Open Door we will still consider your request. As we work together through this process, trust is very important. Please be honest and accurate in your answers. If any information on your application is not true, your request will be denied.

2. If help is approved, most requests are limited to up to $250. In some cases, more help may be given based on the situation and available funds.

3. We are not able to help with cell phones, cable, satellite bills, or rental deposits.

4. After reviewing your request, we may contact you to set up a meeting with one of our financial coaches. Their goal is to walk alongside you, offer guidance, and help you plan for the future.

5. If approved, payments are made directly to the company or person owed (such as a landlord or utility company). We do not give cash or write checks to individuals.

Important Information:
The benevolence fund is meant to be a last source of help. It is used when other options, such as help from family, friends, savings, or other resources, have already been explored. This assistance is meant to be temporary during a time of need.

Please do not include Social Security numbers, ID documents, bank account numbers, or other sensitive information on this form. Submitting an application does not guarantee assistance. Any approved help must be used within 60 days, or it may be canceled. Please respond quickly if we ask for more information so payments are not delayed.
