  - The upload folder can be found at the bottom of the questionnaire.
  - Video(s) can be filmed or recorded on a camera or phone (not expected to be professional quality).
  - If you're interested in serving in multiple areas, please attach the corresponding videos.
3. Once the questionnaire and uploads have been completed, we will send a follow-up email/text.
