At the end of this form you will be required to pay the $25.00 Non-Refundable fee, per person.  

The deadline to guarantee a T-Shirt upon arrival is is Feb 4th.

Please fill out this form as, you, the PARENT.

After filling out this Parent Form and providing payment, you will be redirected to the Student Form to register your student(s).  Both forms need to be filled out for your student(s) to be fully registered.
