Activity Participant Agreement

I acknowledge that participation in the activities (youth programs, basketball, volleyball, pickleball, etc.) at or through the ministry of Devonshire Church may involve risk to the participant and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage

In consideration for the opportunity to participate in the various activities that may be offered through Devonshire Church, I (as the participant or parent/guardian if the participant is a minor) acknowledge and accept the risks of injury or sickness associated with participation in and transportation to and from the activity. I (or parent/guardian) accept personal financial responsibility for any injury or other loss sustained during the activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by the sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to as the “activity sponsor”). Further, I (or the parent/guardian) releases and promises to indemnify, defend, and hold harmless the Devonshire Church for any injury or sickness arising directly or indirectly out of the described activity or transportation to and from the activity, whether such injury arises out of the negligence of the Devonshire Church, the participant, or otherwise.  In the case of communicable diseases, it is recommended that the participant or parent/guardian follow CDC guidelines when considering attendance in such activities and consider the health of others when deciding whether or not to participate.

By registering below, I acknowledge and understand that I am agreeing to this waiver.
