Release Form View Waiver

  - You must read the release/waiver and agree to it before registering. I, as participant/parent or guardian of participant, release First Baptist Church Round Rock, its representatives and or sponsors from all liability, claims or demands for personal injury, sickness or death, as well as property damages and expenses, of any nature whatsoever which may be incurred by the participant while participating in various church activities. In addition, I understand that if I do not have medical insurance, I, said participant/parent or guardian, will be responsible for any medical expenses in the event of a sickness or injury. I understand that there are risks involved in participating in recreational activies and other activities related to the participation in First Baptist Church RR functions. Should it be necessary for the participant to return home due to medical reasons, disciplinary actions or otherwise, participant/parent or guardian will assume all costs for such actions and will assume responsibility of all medical bills, if any.
