The first part of this form is for you, the parent/guardian. Your child's name comes later in the form.
When you reach the end of this first page, you will see a button says "Continue." Click this button to begin adding the information about your children who will attend AWANA. If you have multiple children to register, click the Continue button at the end of each form to register another child.
When you're finished filling out the information for your children, you will automatically be sent to a website where you can pay online for your children's AWANA memberships.
