Parental Consent & Medical Authorization
This permission form is active at any time the below-noted student is attending a regular Grace Students gathering of Grace Polaris Church and/or any of the special events, trips, or activities associated with Grace Students for the 2024–25 ministry year (August 2024–August 2025). We will always seek to inform parents/guardians well in advance of plans that require transportation to take students away from our regular meeting location(s).
My child is voluntarily participating in Grace Students activities, including transportation to and from such activities, with knowledge of dangers involved, and hereby agrees to accept any and all risks of injury as a result of such participation and transportation.
In consideration of my child participating in Grace Students activities, we hereby release and forever discharge Grace Brethren Church of Columbus (DBA: Grace Polaris Church) employees, College of Elders members, and volunteers from any and all actions, causes of actions, claims, and demands from upon or by reason of any damage, loss, or injury, which hereafter may be sustained by the above student in consequence of the above student’s participation in said activities engaged in during the course of their attendance.
All students will be expected to be obedient to all ministry leaders and volunteers at all times within reason. No physical discipline will be used unless in effort to restrain physical misconduct. If our child has behaved with improper inappropriate conduct, I will be notified by a ministry leader or volunteer and agree to come and get him or her at my own expense.
I understand and agree that pictures or video taken during Grace Students events may be used for promotional purposes including but not limited to brochures, videos, social media, and website postings.
In the event that my child needs medical treatment, I know I will be contacted as soon as possible. I authorize the ministry leaders and volunteers to seek medical treatment for my son/daughter. I authorize the doctors, nurses, and hospital to provide any and all necessary treatments. If my student requires emergency transportation, I authorize the responding medical crew to do whatever they deem necessary for the welfare of my son/daughter, even if the child refuses.
