Please find attached the Middle School KidCity application. We would love to have you fill this out. We would also like to ask that at least one of your parents to fill out the adult application at the following link.

Once we receive both applications back, we will get you scheduled for In- Action. In- Action is a "behind the scenes" look at what KidCity is all about and the various areas that you could use your time, treasures and talents!

* Middle Schoolers (6th-8th grade) have the choice of serving in one of the following areas. What is your preference?

* I understand that I am representing Jesus to KidCity Kids. I agree to conduct myself in a manner that will be pleasing to Jesus

* I understand that as a Youth Helper, I am here to assist the adult volunteers in carrying out the mission of KidCity at Pathway.

* I will arrive 30 minutes before service starts and stay until all kids are picked up and toys are put away.

* I will not serve if I have had a contagious illness within the past 48 hours.

* All youth volunteers must have a youth covenant on file and have a name badge before being permitted to serve. I understand that I cannot bring a friend who is not screened to serve in KidCity

* I will be respectful to leaders that I am serving with. I will look to them for guidance at all times.

* I agree to participate in any training given to youth by KidCity leadership, including, for example In-Action. I also agree to read and abide by the KidCity training handbook.

* I understand that if at any time my conduct becomes less than what is expected of me, corrective action steps will be taken. These steps may include verbal warning with instruction on how to conduct myself, written warning, parental notification and, if necessary, dismissal from the ministry.
