Thank you for agreeing to become a Disciple Leaders Network Champion at the Affiliate level.

Please complete all information requested. You can choose to pay by credit card or to "Pay Later" by mailing a check. In either case, once you complete the registration, a confirmation invoice will be emailed to you.

Another email will follow that will provide instructions and deadlines.
