Wednesday Wipeouts - PARENT INFO

Students are invited to Wednesday Wipeouts, a morning of surfing with Josh in Pacifica!

Each week, four students will meet Josh at NCC before heading to Linda Mar to rent wetsuits and surfboards—and then hit the waves together.

For those with experience, we can surf together, and for the newbies, Josh will show you the ropes and teach you what he knows.

Dates: July 1, 8, 15, or 29 (one date only)
Time: 8 am - 1 pm
Cost: $30 for wetsuit and surfboard + $10-15 for lunch at the world's most beautiful Taco Bell after surfing

Liability Release for minors

I give permission for my child(ren) to participate in the activity described above. I, hereby, release and discharge New Community Church and its constituent organizations, agents and employees from any claims for personal injuries or property damage that my child(ren) may suffer as a result of participation, whether or not such injuries or damage are caused by the negligence (active or passive), or any of the entities or individuals mentioned above.

Medical Consent for minors

In case of emergency, I give permission for appropriate care for my child(ren). I agree that in the event there is injury to my child(ren) as a result of their participation, including transportation to and from the event, that I shall be responsible for payment of any hospital, medical, dental or related costs and expenses, and to reimburse New Community Church for any of these expenses paid on their behalf. I consent to any necessary medical care and treatment in the event of injury, including but not limited to, diagnostic studies, anesthesia, and surgical intervention.
