* Transportation to Carson Springs Camp: This is located on the east side of the state in Newport, TN. (Approx. 4 1/2 hr drive.) We are thrilled to announce that a generous donation allows us to provide charter bus transportation to and from camp! We hope everyone can join us on the trip! Let us know your travel plans (you may check more than one box, check all that apply):

* We will be having a Training Week for 5-Day Club the week of June 1-5. Each CYIAer must attend ONE day of that week for training from 8am-3pm. Check as many dates as you are available that week:

* We will be offering two weeks of 5-Day Clubs in various locations and would like to have student teams teaching multiple clubs (2-3) per day during each week. Ideally each team will stay together and serve all day. We will assist in providing meals as they serve. Please check options that best describe the dates/times you are available to serve:

The total fee this year will be $375, which includes the pre-training weekend, training camp and the 5-Day Club kit (which the student will keep), which contains all the teaching materials for the summer. $75 is due by April 1st. If this is an issue, let us know. Fundraising info will be sent to you upon acceptance of your application.
