SWID KIDS CAMP REGISTRATION

PARENT SECTION

You will complete the PARENT information first THEN the CAMPER registration. Please note: Registration is not complete until payment is received.
You can pay through this registration sign up. Some churches will send payments for all campers attending from their church. Please send ONE CHECK PER GROUP. If you pay your church, and your church sends the check, please mark "PAY LATER" when you are completing registration. 

Payment can be mailed by check to address below. 

COST: $220 by April 30, $240 after April 30
